State clearly that this is a Access to Information and Privacy (ATIP) request and describe the records you want as specifically as you can. If you're requesting your own file, include your full name and any case, claim, or file number the institution uses. Add a return address so they can reply, and keep a copy of what you sent and the date — you'll need both if you have to appeal a refusal or a missed deadline.
